Requests can be submitted in person, by mail, or online. A request for an official transcript made in writing must include all applicable names, social security number, date of birth, complete address, student’s signature and purpose of request. The request should include student’s full name, valid ID with social security number, dates of attendance, current addresses and the complete address to which the transcript should be sent. Transcripts cannot be produced for students that have a financial obligation to the university. NOTE: Effective August 13, 2011 the transcript fee increased to $5.00. This fee should be paid in the Bursars Office.
*****Official Online Transcript Request Form*****
NOTE: This link is for students who attended the university from 1985 to present. You must use your Banner ID Number or Social Security Number when ordering online. Students who attended the university prior to 1985 must make their request in person or by mail for an official transcript. You must provide a forwarding address to have your transcript mailed; if not, it is assumed that your transcript is to be picked up in the office.***
Can someone else pick up an unofficial or official transcript for me?
Yes. If someone else is picking up the transcript they will need to have a picture ID and a letter authorizing them to pick up the transcript. Also include a telephone number in case there is a question regarding the request.
Does the University provide same-day pickup for transcripts?
No, not at the present time. Official transcripts cannot be produced immediately upon request. We suggest that students allow one week for the transcripts to be processed but most requests are completed within three working days of their receipt. Allow two weeks for the transcript process if it is at the end of the semester or after graduation.
How can I check on the status of my transcript request?
You may check on the status of your transcript by contacting the office at (256)-372-5254.
