Electronic Personnel Action Form (EPAF)

Electronic Personnel Action Form is an online form originated by the department to communicate an employment decision (personnel action) about an employee to the Human Resources and Payroll.  All employment decisions require the attention of Human Resources, and require the employees' Banner record to be updated.  So, the department is required to initiate and submit an EPAF transaction that specifies the nature and details of the employment decision.

Types of EPAF Users (Roles and Responsibilities)

  1. Originator - create and submit EPAF transactions.  
  2. Approver - process EPAF to review, approve, acknowledge and return for correction.
  3. Apply (Human Resources) - review, approve and apply EPAF to Banner HR.

Logging into the EPAF system using Self-Service Banner

  • Login into Self-Service Banner:  Enter your User ID and PIN, click Login.
  • Click on the Employee Tab, Electronic Personnel Action Form link.

Departments will use the EPAF process to provide the employment data necessary to process the following EPAF Approval Categories:  Click the link below for Step-by-Step Instructions and Samples.

  • Rehire of Full-time, Part-Time Staff and Adjunct Faculty 

  • Hire and Rehire of Graduates

  • End a job assignement (excluding full employee separation)

  • Overloads and Supplements 

Register for EPAF Training

EPAF Training is designed to provide participants with an overview of the EPAF Process using Self-service Banner (SSB).  Training sessions are offered quarterly or as needed to accommodate the end users. Hands-On EPAF Training sessions are approximately two hours and will provide the user with technical skills and procedural knowledge to manage the EPAF process.

Three Types of EPAF Training:  Schedule for EPAF Training

1.  Hands-on EPAF Training - Refresher course to gain better understanding of the EPAF Process.

2.  Walk-in Computer Lab - Working sessions for assistance with entering EPAF
into the system.
