immunization

Please click Immunization Records to upload the required documents.

Alabama A&M University requires all first-time-entering students to submit certain health documents.  These documents include proof of specific immunizations (MMR, Meningococcal ACWY (for those under 21 or living on campus), a TB Screening Questionnaire (may lead to further testing), and a History & Physical exam.  All health documents are submitted electronically, and a hold may be placed on a student's account if requirements are not met.

Required immunizations and screenings

All new students must provide documentary evidence of immunity or vaccination for the following:

            • Measles, Mumps, Rubella (MMR): Two doses of the MMR vaccine or proof of immunity via a blood test (titer).
            • Meningitis (MenACWY): One dose received after age 16 for all students under the age of 21 and for any student living in campus housing, regardless of age.

Tuberculosis (TB) Screening

            • Students must complete a Tuberculosis Risk Assessment Form. Those who screen positive on the assessment will be required to undergo further testing, such as a Chest X-ray, TB skin test, or a blood test.

History & Physical Exam

            • Students must complete a Tuberculosis Risk Assessment Form. Those who screen positive on the assessment will be required to undergo further testing, such as a Chest X-ray, TB skin test, or a blood test.

Important information

            • Submission: Students may upload Proof of Immunization Form and History & Physical via the Student Health Portal. The portal may be accessed by clicking the link or scanning the QR code found on the AAMU Health & Counseling Services Website. Acceptable documentation includes an official certificate, a photocopy of a certificate, the Immunization Form, or written documentation from a physician.
            • Healthcare Provider Completion: The form must be completed, signed, or stamped by a licensed healthcare provider.
            • Processing typically takes 48-72 hours, after which the student will receive confirmation or a message explaining any issues with the submission.
            • Delays: The University has a final deadline for immunization compliance. If the requirements are not met, a hold may be placed on a student's account.
            • Exemptions: Due to Alabama law, religious exemptions are not available for institutions of higher learning.

Important notes

            • Failure to submit the required documentation by the deadline will result in a hold being placed on the student's account, preventing class registration.
            • Distance learners are not required to provide proof of immunization, but the requirements apply if they enroll in any on-campus courses.
            • The AAMU Health and Counseling Center offers all required vaccines and TB screenings, which are billable to insurance.
